Both are polite ways to ask for something. They are almost exactly the same in meaning, but I suppose saying "could" is slightly more polite. Still, they are almost exactly the same in US English and it doesn't really matter which you use.

Could is the past tense of can, therefore, "I'd appreciate it if you could..." would mean, "I'd appreciate it if you had the ability to..."
Would is the past tense of will, therefore, "I'd appreciate it if you would..." would mean, "I'd appreciate it if you will..."
Could is simply stating that you can do something/have the ability to do something.
Would is stating that you can AND will do something.
